Watch for your 1099-HC Tax Form in the Mail

Blue Cross Blue Shield (BCBS) of Massachusetts and Health Plans Inc. (HPI) recently mailed Massachusetts 1099-HC tax forms to plan members who were enrolled in 2018 health insurance benefits through UMass Memorial Health Care. This form is required for filing Massachusetts state income tax returns! New this year: You can request an electronic copy of the 1099-HC form from your carrier. Simply log onto your account on the BCBSMA or HPI website to submit a request for your electronic copy of the tax form. What You Need to Know about 1099-HC and 1095-C Tax Forms

If you need a duplicate copy, contact Blue Cross Blue Shield at 800-932-8323 or Health Plans, Inc. at 844-998-6642. As is required of large employers, UMass Memorial Health Care sent federal 1095-C tax forms to employees who were eligible for health care benefits last year. This form is not required for filing income tax returns; simply keep it with your other tax documents.
